Incident Reporting
Mozaic follows all applicable New York State requirements for identifying, reporting, and reviewing incidents to ensure appropriate follow-up, corrective actions, and protection of individuals.
Examples of Reportable Incidents
Examples of reportable incidents may include:
Injuries or medical emergencies
Allegations of abuse or neglect
Significant behavioral events
Events that impact the safety or well-being of an individual
What Happens After an Incident Is Reported
All reported incidents are reviewed to ensure appropriate action is taken. This may include:
Immediate response to ensure safety
Notification of appropriate parties
Review and investigation (when required)
Implementation of corrective actions
Oversight & External Agencies
Mozaic works closely with state oversight agencies to ensure all incidents are handled in accordance with regulatory requirements.
