Mozaic is committed to ensuring the safety, rights, and well-being of the people receiving services.


Incident Reporting

Mozaic follows all applicable New York State requirements for identifying, reporting, and reviewing incidents to ensure appropriate follow-up, corrective actions, and protection of individuals.


Examples of Reportable Incidents

Examples of reportable incidents may include:

  • Injuries or medical emergencies

  • Allegations of abuse or neglect

  • Significant behavioral events

  • Events that impact the safety or well-being of an individual


What Happens After an Incident Is Reported

All reported incidents are reviewed to ensure appropriate action is taken. This may include:

  • Immediate response to ensure safety

  • Notification of appropriate parties

  • Review and investigation (when required)

  • Implementation of corrective actions


Oversight & External Agencies

Mozaic works closely with state oversight agencies to ensure all incidents are handled in accordance with regulatory requirements.
