Incident Reporting
Mozaic follows all applicable New York State requirements for identifying, reporting, and reviewing incidents to ensure appropriate follow-up, corrective actions, and protection of individuals.
Jonathan’s Law
Jonathan’s Law provides individuals receiving services and their families or legal representatives with notification and access to information regarding reportable incidents.
Under Jonathan’s Law, qualified persons may:
Receive notification when a reportable incident occurs
Receive a written report outlining actions taken
Request access to certain records related to the incident
Requests for records must be submitted in writing.
