Step #2: Care Coordination Selection

When all questions are answered, you will select your Care Coordination provider and let OPWDD and the providers know whom you have chosen.  We will help guide you with any additional questions or concerns you may have.

Step #3: Plan Development 

With the assistance of your Care Coordinator, you will develop a plan to cover all your needs. Mozaic is here to help you and your family.
