Habilitation
Community Habilitation works with the individual to identify specific goals to increase, expand, and maintain skills to become as independent as possible. Skill areas include but not limited to, budgeting, socialization, travel training, community involvement, self-care, safety skills, exploration of interests and hobbies, accessing community and generic resources, etc. Community Habilitation is provided in the individual’s home and community to best practice the skill areas identified. We provide Community Habilitation to all ages.
Respite
Respite is a service to help provide families a break from the ongoing care required for their loved one. The service provides group activities for socialization and recreation opportunities in the community. Respite also offers various trips with continual staffing support. Recreational and group activities are offered based on people’s interest and hobbies they favor.
Self-Direction for Respite
and Community Habilitation
We offer a self-direction option for supported persons and their families who wish to have a higher level of choice and control within these services. Through this option, supported persons and their families partner with Mozaic to identify and hire their own Direct Support Professional, dedicated exclusively to them. With guidance from Mozaic, the supported person and/or their family members oversee day-to-day supervision, including activities, scheduling, and service progress.
Social Skills Group
Social Skills is a group which means throughout the year to practice strong social and interactive skills. These skills help enhance the individuals’ ability to communicate with their peers, family members, doctor’s, emergency personnel, etc. The group also focuses on how to overcome problematic situations in which communication maybe hindered. The Social Skills group allows the participants to role play and rehearse various situations with staffing support. This increases the person’s confidence in his/her own socialization and interaction skills and abilities. (Yates County only)
Individualized Support Services (ISS)
ISS is a financial assistance program designed to support supported persons in achieving independent living. Available to those ages 18 and older, the program can cover part or all of a person’s rent based on their monthly income until approval is received for Section 8 or HUD housing programs. ISS may also assist those moving into their own apartment by providing funding for essential household items such as furniture, linens, and kitchenware.
(Available in Cayuga, Onondaga, Oswego, Tompkins, Cortland, Seneca, Ontario, Wayne, and Yates counties.)
